multiple myleoma – kappa value increases and M-band decreases

    Hi, It would be gteat if some one reply to my query: My father was diagnosed with Stage 3 multiple myeloma. He has received 10 cycles of Bortezomib, Endoxan and dexamethasone.

    Below is the data of his medical test reports. What do the results indicate?. any change required in medicines?
    1. Kappa Free Light Chain
    At diagnosis – 42.70
    After 6 treatment cycles – 789
    After 10 treatment cycles – 1470

    2. Lambda Free Light Chain
    At diagnosis – 11.30
    After 6 treatment cycles – 10.20
    After 10 treatment cycles – 7.74

    3. Kappa/Lambda Ratio
    At diagnosis – 3.8
    After 6 treatment cycles – 77.35
    After 10 treatment cycles – 189

    4. Protein Electrophoresis Serum (M-Spike)
    At diagnosis – 2.43
    After 6 treatment cycles – 0.71
    After 10 treatment cycles – 0.75

    5. Immunoglobulin level values
    Immunoglobulin (One month back (means after six cycles)) mg/dl
    IgG, Serum – 518
    IgM, Serum – 33
    IgA, Serum – 537

    Immunoglobulin (current values (means after ten cycles)) mg/dl
    IgG, Serum – 469
    IgM, Serum – 28
    IgA, Serum – 593

    6. CBC / KFT is almost in normal valueas

    Hi Nihal

    Sorry to hear that your father has myeloma. I would have thought that the haematology team or your GP would be best placed to explain the significance of these results.

    The problem is that you might not have the complete picture, so could jump to the wrong conclusion?

    Hello Nihal,

    Sorry to hear about your dad.  I am no doctor,   just a sufferer of lambda light chain (LC) MM.  To my untrained eye it looks like your dad has kappa LC MM.  I understand if you have LC MM, you tend to have just one type (kappa or lambda) but not both.

    As David says above, you have to speak with the medical team and ask them the significance of rising kappa LCs and the meanings of all the other test results.  Sorry I can’t help any more.  Best wishes to your dad.
