Hi fellow travellers.
While seeing the doc today she suggested that I apply for a Blue Badge as my heart is affected and I get very breathless. My problem is – and I hope someone can shed some light – she said to phone DVLA and get the single sheet doctor application. I have looked and looked at the site and can't find anything other than the 24 page application form. Can anyone help please?

Try Mcmillan they gave me a form and the Mcmillan nurse in the hospital signed it for me you doc will probably sign yours as he suggested it its a real help if if you are feeling unwell a lot of places charge in carparks but you usually get a space you can also park on yellow lines upto 3 hours good luck

I have just renewed my blue badge (they last three years) and still had to fill in the 70 odd pages lol, never knew about a single page one all I know was she said (lady in blue badge dept) that if I have a Mac Nurse it will come through faster as it was only a phone call for them to make (i dint have a mac nurse and still dont have one) so it was a letter to my Dr for him to confirm I had MM, second one was fill in same form and a Nurse at the blue badge Dept said yes if he had it three years ago (confirmed by his Dr then he still has it) ha ha.
When you get it dont forget your free bus pass yep free, and mine covers me for free travel on the train (I live in Scunthorpe) from Cleathorpes to Doncaster and those in our area.
Good Luck with it all its well worth its money even though the Hospitals still make you pay to park, but I park on Yellow lines outside the hospital and Bu&&er they payment he he

I applied for Henry's blue badge from our local Council – they sent me the forms and we filled them out (not many pages) – we then had to take the form to our doctor for him to complete – send it off and hey presto the badge arrived. It has been a godsend. We do use it at the hospital but we also get free parking as he attends the macmillan unit.

My experience was very similar to Sarah's… form at home, on to my GP and Badge in the post within a week. My renewal was a very similar process only this time we had to drop the form into a local council help point where they rubber stamped the form and the new badge arrived within 48 hours. Easy Peasy.:-D
You still have to pay at our hospital in the main car parks but there are plenty of reserved parking spaces for Blue Badge holders… usually within a 100 metres of major clinics and entrances.:-)
It is a really useful scheme and well worth pursuing, even if you are capable of limited mobility.8-)

I also cannot find a DVLA single sheet
Macmillan did not offer me one , gave me local authority number to call
My Daughter works at the Ribble Valley Council offices , the do not have Henry Blue Badge forms , she gave me an 0845 number to call , probably same number as Macmillan
My next stop will be to see my Doc & call at local hospital which has a Macmillan office
Not sure what I am doing wrong , but a. Badge would be really useful

When I got my blue badge, it was from the local council, not from DVLA. It was very straightforward as I recall.
I don't think this is the way I did it, but I've just found the direct.gov website for applying: https://www.gov.uk/apply-blue-badge
Good luck! It has made such a difference for me, being able to park near places and often for free.

Thanks to everyone for their advice. I spoke to the local council today and apparently the one sheet claim form is no longer available. They were very helpful though and are sending me the forms. I really hope I am successful as it will make such a difference.

The Macmillan nurse at our hospital filled my Mums application in, which was really helpful. She received her badge in less than 2 weeks, and already it has made a difference to our little outings, being able to park nearer to places is great for Mum as shes still very tired post SCT.

Im Paul's wife Gayle and I work in the Blue Badge section in our local authority.Give your local council a tel call to send you an application form.A new national scheme started April this year and each authority does some bits differently.If you receive DLA high rate for mobility the badge is an automatic acceptance. If you dont have this allowance then complete the parts for walking difficulties. We send to the GP ourselves if necessary some authorities will ask you to take the form yourself for GP signature. We ask if you can provide evidence so maybe your nurse or consultant could give a letter supporting your application( your authority may be different you would have to check the guideline sheet they send). The Macmillan welfare section can help you complete the form or Age concern.

Hi Gayle, thanks so much for your advice. I did contact the local council and the lady I spoke to there was really very helpful. She is sending the form through and told me which sections to complete. She even offered to go through it with me should I need her to. I am not eligible for an allowance as working for Loughborough University I am still being paid. I think I got thrown off track with the consultant saying to ask for the single sheet GP form, which apparently is no longer in existance. After venturing into town today I can see how very helpful a blue badge would be as I couldn't manage to go to all the places I would have liked as they were too far apart to walk. I am really struggling with the dependancy aspects of this illness, being so independant previously, and anything that can ease that a little would be so welcome.

I got my blue badge through my local council and found it straight forward. I have the higher rate DLA so I qualified automatically. I was also entitled to a bus pass as well. I'm 54 😉
